During chemotherapy, many people are asked for an electrocardiogram (ECG) alongside their bloodwork, or an echocardiogram every few months. Some anticancer drugs can place a burden on the heart, and these tests are a way of looking before symptoms appear. This range of effects is often grouped under the term cardiotoxicity, and how closely it is monitored depends on the drug, the cumulative dose, age, conditions such as high blood pressure or diabetes, and any past radiation to the chest. Being tested does not by itself mean something is wrong; it is usually part of keeping treatment on track safely.

It helps to know that these tests look at different things. An ECG records the heart's electrical signals over a few seconds — heart rate, rhythm regularity, and intervals such as the QT interval. An echocardiogram looks at how well the heart actually pumps, most often reported as the left ventricular ejection fraction (LVEF), along with the valves and the sac around the heart. Blood tests for cardiac markers may be added. So a flagged ECG does not automatically mean the pumping function is impaired, and a clean ECG does not confirm that function either.

Results can also differ from one cycle to the next, because an ECG captures the body as it is in that moment. After a stretch of vomiting or diarrhea, electrolytes such as potassium and magnesium can shift and affect rhythm. Dehydration, anemia, fever, pain, or simple nervousness can raise the heart rate or make it look irregular. Medicines used alongside treatment — some anti-nausea drugs, certain antibiotics and antifungals — can lengthen the QT interval. Technical factors matter too: electrode placement, shivering, and breathing can change how a tracing looks. This is why wording such as suspected or possible is closer to a request for another look than to a diagnosis, and why it may lead to a 24-hour Holter monitor, an echocardiogram, or further blood tests.

If the next tracing is normal, it becomes more likely that the earlier finding was temporary — but the record still matters. Once an item has been flagged, it becomes a comparison point later on, so keeping a single page listing which tests were done, when, and what they showed is useful if you change hospitals or clinicians.

At home, watch for change rather than numbers alone. Palpitations that continue for several minutes at rest, dizziness or fainting, new breathlessness on stairs or level ground, needing extra pillows to lie down at night, swelling in the ankles or shins, and sudden weight gain over a few days are all worth reporting without waiting for the next visit. Chest pressure or pain, fainting, and severe shortness of breath need emergency care. A short daily note — pulse and blood pressure taken at the same time each morning, weight, and when symptoms started, how long they lasted, and what you were doing — makes the conversation in clinic far more precise. Smartwatch recordings are a reference point, not a diagnosis, but saved screens can be a useful starting place.

An appointment that ends in a minute is not a bad sign in itself, though a short visit rewards preparation. Narrow your questions to three in order of priority. Describe new symptoms as when they began, how often they occur, and what they now prevent you from doing. Bring a list of all medicines and supplements. Before you leave, ask whether any test should be repeated before the next cycle, and which symptoms warrant calling between cycles — that single answer makes decisions at home much easier. New sensory changes, such as cold or tingling fingertips, are worth logging by cycle and severity as well, since they inform later discussions about dose and scheduling.
