When a pathogenic variant in the BRCA1 or BRCA2 gene is identified, one option that may be discussed is risk-reducing salpingo-oophorectomy (RRSO) — removal of both ovaries and fallopian tubes to lower the risk of ovarian and tubal cancer. Because there is no reliable screening test that catches ovarian cancer early, prevention rather than detection becomes part of the conversation for people at high risk. The surgery is usually discussed before natural menopause would occur, so the benefit of risk reduction is weighed alongside the changes that come with menopause arriving earlier than expected.

The first thing worth understanding is that surgical menopause differs from natural menopause mainly in speed. Natural menopause unfolds over several years as hormone levels gradually decline, while removing both ovaries causes estrogen to drop sharply within days. For that reason symptoms can appear sooner and feel more pronounced. Commonly described experiences include hot flashes, night sweats that interrupt sleep, lighter or broken sleep, mood swings, difficulty concentrating, and vaginal dryness, discomfort with intercourse or urinary irritation — often grouped together as genitourinary syndrome of menopause. The intensity varies widely from person to person, and some people notice relatively little.

Over the longer term, attention usually turns to bones and blood vessels. Estrogen helps limit how quickly bone is lost, so when ovarian function stops at a younger age, bone density may decline faster, and clinicians monitor for osteopenia and osteoporosis. Cholesterol and other lipid levels, blood pressure and body weight can also shift after menopause, so cardiovascular health is followed as well. These are not certainties but items placed on a watch list because the risk rises somewhat.

Management is generally discussed along three lines. The first is menopausal hormone therapy. For someone who has surgical menopause at a young age without a history of breast cancer or other contraindications, hormone replacement may be considered for symptom relief and long-term health — but the decision depends heavily on personal breast cancer history, family history, clotting risk and whether risk-reducing breast surgery has been done. This particular question should be answered by clinicians who know your records, not by internet accounts or another person's experience. The second line is non-hormonal options, including medications that may ease hot flashes and topical moisturizers or lubricants for vaginal dryness, again chosen after consultation.

The third line is lifestyle and baseline testing. Weight-bearing activity such as walking or light jogging, along with resistance exercise, is generally recommended for maintaining bone and muscle; adequate calcium and vitamin D, not smoking, limiting alcohol and keeping regular sleep are also encouraged. Having a bone density scan (DXA), a lipid panel and a blood pressure reading around the time of surgery gives a personal baseline, which makes later changes far easier to interpret. Follow-up intervals are then set by your care team.

Questions worth writing down before the consultation include: why this timing is being recommended for me, whether hormone therapy is an option or something I should avoid, which baseline tests I should have before surgery, whom to contact if menopausal symptoms become difficult, and what the plan will be regarding the uterus and the breasts going forward. Taking time to ask again rather than deciding quickly often leaves people more settled afterward.
