When radiation therapy is new, the smallest questions often feel the most unsettling: Does the machine move around me? Do I have to undress? Will it hurt? Understanding the flow ahead of time makes those moments far less alarming. Radiation treatment is not a single event on the day the beam is delivered. It usually splits into a preparation stage, where a precise aiming plan is built for your body, and the actual delivery of the radiation.
The first step is usually called simulation, or CT simulation. This scan is not meant to diagnose disease again; it captures the blueprint a team needs to calculate exactly where and how much radiation to deliver. To expose the treatment area accurately, clothing may be lifted or removed, not to examine you, but so that the same position can be reproduced every single time. Small dots or lines may be drawn on the skin, and sometimes a tiny permanent mark (a skin tattoo) is placed so the setup stays consistent.
For the chest or abdomen, breathing coaching is sometimes added. Because organs shift slightly with every breath, techniques such as holding the breath briefly or delivering radiation in time with the breathing cycle (respiratory gating) help protect healthy tissue. Practicing beforehand makes the real session smoother.
On treatment day, you lie on the couch in the same position set during preparation. Depending on the area, clothing may only be lifted partway, or the same conditions as the planning day may be matched. A machine called a linear accelerator (linac) has a large arm (the gantry) that rotates around the body, aiming radiation from several directions. Unlike a CT, you are not enclosed deep inside a narrow tube; the space is usually open. Importantly, you feel nothing as the radiation is delivered, no heat and no sting, so your only job is to stay still.
Being told the treatment is just once may mean a single, relatively higher dose is given in one session instead of being split over many days. The number of sessions and the approach depend on the goal, whether easing pain or targeting remaining disease, so it is worth asking your team why it is a single session and what the next plan is. The beam itself often takes only a few minutes, though positioning and checks can add time.
