When people start chemotherapy, many wonder whether a working treatment will bring an immediate, noticeable change — a lump that shrinks under the fingers, or pain that eases right away. Because the body signals distress quickly when side effects appear, it feels natural to expect that improvement should be just as easy to sense. In reality, however, judging whether a treatment is effective by feeling alone is difficult.

A common misunderstanding is that stronger side effects mean the drug is working better. Nausea, fatigue, and hair loss occur because chemotherapy also affects healthy cells, and they have no direct relationship to how much a tumor is shrinking. Mild side effects do not mean the treatment is failing, and severe side effects do not prove it is succeeding.

Symptoms you can feel or touch — a lump near the skin, or pain from bone involvement — sometimes do improve early in treatment. Easier breathing or reduced pain can be a welcome sign. Yet these visible changes are only part of the picture. Disease deep inside the body often changes without any sensation we can notice.

For this reason, care teams confirm response through tests rather than feelings. Typically, imaging such as CT or MRI is repeated every two or three cycles to measure changes in tumor size, alongside tumor markers and a physical exam. Internationally, standardized criteria (such as RECIST) describe response in categories like complete response, partial response, stable disease, and progression. What matters most is comparing several points in time, not a single scan.

Not feeling an immediate change is not a reason for alarm. Living comfortably without obvious symptoms can itself be a sign that the disease is being controlled. At your next visit, it can help to ask how your response is being tracked, when the next scan is planned, and which symptoms you should report — turning an anxious wait into a calmer, more informed one.
